It is used when ordinary documents cannot prove the relationship – records are missing, names are spelled differently, or civil registration was never completed.

A usable affidavit identifies each person by full name and date of birth, states the exact relationship, and explains how the person swearing it knows it to be true.

How to get an affidavit of relationship

Ask the receiving authority first. Many embassies, courts and registries publish exactly what the statement must cover, and a document that misses one point is sent back.

The affidavit should name the person swearing it, identify each person concerned in full, state the relationship precisely, and explain how the signatory knows it – “lived in the same household for twenty years” carries more weight than a bare assertion.

Attach copies of any supporting documents. Two affidavits from people who know the family independently are stronger than one.

No. The USRAP Affidavit of Relationship, Form DS-7656, is part of the US refugee admissions programme and must be completed in person with a resettlement agency that has a cooperative agreement with the US Department of State.
